in Social Work or related field preferred with 2 (plus) years experience preferred.
Long Term Care experience needed.
Must be familiar with the M.
D.
S.
and state/federal guidelines for Skilled Nursing Facilities.
Applicants must posses skills to: * Conduct intake interview and assessments * Assist residents in completion of Advanced Directives and guardianship as needed * Participate in the development and review of care plans * Facilitate supportive treatment intervention to maintain or enhance resident's capacity for independent functioning * Assist as a liaison with outside agencies as needed to facilitate communication and services * Assist in coordination of discharge planning services * Provide support for behavioral management * Accept and follow up on grievance issues reported * Maintain close relationship with residents and their family members * Maintain concise comprehensive case records by following established agency policies and procedures * All other duties as assigned Job Type: Full-time Benefits: * Dental insurance * Health insurance * Life insurance * Paid time off * Referral program * Vision insurance Schedule: * 8 hour shift * Day shift * Monday to Friday Work Location: In person
