Description:
Transition is seeking a full-time Office Assistant & Coordinator of Volunteer Services who will be responsible for planning, coordinating, and managing all volunteer program activities within Transitions.
This individual will support the volunteer design, build a robust team, and support all levels of connectivity at our growing office locations.
The Team Assistant is responsible for the team schedules to ensure visit frequencies are maintained, assist in preparation of IDG team meeting needs, auditing charts for required items and assisting in follow up as well as support the team from the back office.
The Team Assistant is also responsible for patient audits as necessary along with receptionist duties and general office support.
Duties and Responsibilities
Verify accuracy of data entry from completed admissions, and ensure that all information is received at the start of care.
(H&P, start of care orders, emergency contacts, POLST, POA paperwork, etc)
Assist staff with requests as needed-scheduling, faxing, phone calls, requests, etc
Perform data entry of patient information, including changes in levels of care, discharges and any additional information as needed
Upload items into the patient chart to ensure medical records are kept current
Submit all discharge paperwork to the attending physicians upon patient discharge/revocation.
Ensure all CTI (Certification of Terminal Illness) orders are signed/received upon admission
Submit coroner forms upon admission (county specific)
Maintain and distribute daily census
Schedule Face to Face encounters with NP/MD and maintain spreadsheet for auditing purposes, as needed
Assist QA department with continuing quality improvement program
Assist in the tracking, uploading and receiving of all signed orders via Forcura/Manual as needed
Ensuring that plan of cares are distributed to attending physicians as well as facilities
Updating demographic & facility information as requested
Process missed visits, visit time change requests, and coordination notes
Other audits and duties as assigned by leadership
Assist in the preparation of the IDG meetings with QA RN and RDN
Scribe for the IDG team meetings to ensure appropriate documentation of the meeting
Ensure all new hires have supplies prepared for them upon start dates (coordinated with HR & IT)
Maintain temperature logs on refrigerators that contain medications
Develop and maintain volunteer training and orientation program.
Recruits, educates, and selects volunteers from at least three (3) recruitment sessions a year.
Establish a need for volunteer services with the patient and family/caregiver and entire hospice team.
Supervise and evaluate volunteers in a timely and appropriate manner.
Maintains monthly statistics.
Maintains current personnel files.
Reevaluate volunteer program as necessary.
Coordinate support/education meetings.
Provides information to individuals and community groups about hospice volunteer services.
Participates as needed in the budget preparation for program development.
Actively participates in quality assessment performance improvement teams and activities.
Accepts and performs other related duties and responsibilities.
Requirements:
QUALIFICATIONS
Education and Experience
College Degree in applicable area of study - Bachelor Degree preferred
Advanced degree in Human Services preferred or equivalent work experience.
2+ years of experience in health care setting.
Hospice and/or volunteer experience preferred.
Comprehensive knowledge in the hospice philosophy.
Ability to organize and develop volunteer hospice personnel for both patient and organizational needs.
Ability to manage a group of individuals providing volunteer time and who will be considered employees.
